Dr. Coni Dennis, DNP, RN, NE-BC, is Senior Vice President and Chief Clinical Officer at CCS. She leads the company’s clinical strategy to advance quality, evidence-based care for people living with cardiovascular-kidney-metabolic conditions. With more than 20 years of experience in nursing, healthcare leadership, and population health, she drives enterprise-wide initiatives that improve outcomes, reduce costs, and strengthen patient confidence in managing chronic conditions.
What does collaborative care mean to you?
To me, Collaborative Care means breaking down silos so patients do not fall through the cracks. It is about aligning all the disparate parts of the healthcare ecosystem to help the patient make sense of it all, and surrounding them with support that is simple, connected, and effective. When done well, collaborative care delivers measurable results: fewer gaps, higher adherence, lower costs, and healthier, more confident patients. Just as important, it builds trust and confidence in patients, and strengthens the relationships among colleagues who know that together, we can achieve more than we ever could alone.
