LivingConnected Sunset FAQs

As the year comes to a close, we want to thank you for being part of the LivingConnected program. The program will officially end on December 31, 2025, as the partnership between CCS and your employer concludes. We’ve truly valued the opportunity to support your health journey. Whether it was through coaching sessions or the app, we hope LivingConnected made a meaningful difference for you. Please see the FAQs below. Program Logistics

When does my LivingConnected program end?

Your LivingConnected program will end on December 31, 2025. 

Why is the program ending?

I know this is unexpected.  Unfortunately, the partnership between CCS and your employer will not continue into the new yearIf you have other questions, we recommend reaching out to your employer’s benefits teamI am here to help you with your next steps. 

Can I continue in LivingConnected on my own?

Unfortunately, this program is only offered through employer participation and cannot be continued individually.

Will someone from CCS reach out to me?

If you’ve used your device recently (in the last 30 days) or had a support call in the last 90 days, our team will call you to review your options. If you have any questions, feel free to call 800.966.2046, Monday through Friday from 9:00 AM – 7:00 PM EST.

What if I don’t respond?

If we don’t hear from you, your program participation will automatically end on December 31, 2025.  This will include access to the LivingConnected app.

I have questions. Is there someone I can call at CCS?

Yes. You can reach our team at 800.966.2046, Monday through Friday from 9:00 AM – 7:00 PM EST. 

Monitoring and Devices

Can I still use my meter?

Yes, you can continue using your current meter and strips while you have them. Once you run out, you’ll need a new prescription for a new meter and supplies from your doctor to continue testing. The supplies can then be filled at your local pharmacy. 

Will CCS still receive my readings?

No. After December 31, 2025, CCS will no longer receive your glucose readings. That means our clinicians will not be able to see your results or provide the same monitoring and outreach we do today. 

What about the 24/7 monitoring?

The 24/7 monitoring service through CCS will also end with the program. If you experience a critical low or high blood sugar after December 31, 2025, you’ll need to follow your doctor’s instructions and reach out directly to your healthcare provider or emergency services if needed. 

What about the LivingConnected App?

Access to the LivingConnected app will also end on December 31, 2025. If you would like to have a record of your data, you can download your Health Report before the program ends.
